Lead Care Coordinator is a leadership role within the Wraparound Milwaukee program focused on providing strength-based, trauma-informed, family-driven, community-based care. This position supports Care Coordinators in the REACH (Reaching, Engaging, and Assisting Children & Families) - Crisis program and delivers individualized care coordination for youth and young adults.

Job Responsibilities Provide leadership and role modeling of the Wraparound process for Care Coordinators.
Assist Supervisors with development of care coordinators, including mentoring and training, problem-solving and feedback, identifying informal and natural supports, and coaching to follow best practices and program policies.
Provide hands-on training in the community with new Care Coordinators for the first 6 months of employment.
Attend Team/Plan of Care Meetings and Court Hearings to coach staff on the Wraparound process, policies, and procedures; ensure follow-through on policies with existing staff.
Collaborate with Supervisor to utilize reports and data from Milwaukee County Behavioral Health Services to improve care for youth and families.
Maintain a caseload of approximately 6 youth and families.
Contact enrolled youth and families within 48 hours of enrollment and schedule an initial visit within 5 business days to introduce self and available supports.
Schedule and attend required face-to-face visits and facilitate Child and Family Team Meetings for treatment planning per program guidelines and deadlines.
Within the first 30 days, coordinate the initial Child and Family Team meeting and develop the initial Plan of Care based on identified strengths and needs, including a comprehensive 24-hour Crisis Plan.
Monitor service quality and act as liaison to seek or develop new services/resources as needed.
Advocate for youth and families across settings (home, education, court, community).
Attend all scheduled court hearings as a representative of the REACH-Crisis program and explain available supports.
Identify and secure community resources with the Team and modify Plans of Care accordingly; document changes in Avatar as needed.
Provide or arrange transportation for youth/young adults to appointments and crisis services; assist in identifying natural supports and sustainable transportation plans.
Provide or secure crisis/emergency services as needed, through in-person contact, phone availability, or on-call systems.
Complete all necessary paperwork in a strength-based manner per agency requirements (e.g., Plans of Care, discharge summaries, change of placement forms, provider notes, etc.).
Maintain accurate client information in Avatar and coordinate with other involved individuals.
Report emergencies or critical incidents to Wraparound Milwaukee (refer to policy).
Qualifications Required Qualifications
Must possess at least a bachelor’s degree in a relevant field of education or human services.
Minimum of one year of experience providing mental health services; experience with children or adolescents in mental health is required, OR 20 hours of relevant training within 3 months of hire in addition to DHS 34 requirements.
At least one year of Care Coordination experience with Wraparound Milwaukee; promotion to Lead requires approval in writing by the Wraparound Milwaukee Program Manager (or designee).
Lead Care Coordinators must be certified as a Care Coordinator by Wraparound Milwaukee prior to eligibility for the lead role.
Knowledge, Skills & Abilities Mental Health/Human Development: working knowledge of mental health needs and positive child/youth development.
Trauma-Informed Care: patience and understanding of traumatic life situations.
De-escalation Skills: ability to manage escalating youth or parents calmly and professionally.
Technological Aptitude: comfort with electronic health records (Avatar) and Microsoft Office tools.
Time Management: ability to maintain schedules, meet deadlines, and multitask.
Multicultural Sensitivity: professionalism and strong interpersonal skills when working with diverse populations.
Communication: clear written and oral communication with clients, families, supervisors, and partners.
Problem-Solving: critical thinking and solution-oriented approach in a fast-paced environment.
Adaptability: ability to manage change and work independently or as part of a team.
Teamwork and Professionalism: collaborative teamwork with integrity and ethical boundaries.
Other Job Information Physical Demands : Duties may require activity during client interactions; driving is required. Reasonable accommodations may be made for disabilities.
Work Environment : Office-based with caseload management, community outreach, and occasional evenings/weekends; driving required in some cases; typical workweek around 40 hours.
