Health-Related Social Needs (HRSN) Case Manager

Summary
Provide case management services to guests residing in Emergency Lodging. Coordinate access to and support guests in obtaining resources needed to support recovery, including medical care, housing, employment, childcare. Maintain working relationships with community resource partners. Hours may vary and the schedule may include weekends, evenings, and holidays.
Key Responsibilities
Ability to work independently, professionally and collaboratively with staff from multiple
Documentation of client of case management activities in EHR
Must be able to maintain strong boundaries
Ability to work collaboratively with outside agencies to assist client in meeting needs
Ability to work with diverse populations and cultural backgrounds
Ability to work with difficult individuals, including those with a criminal background
Ability to focus on and reinforce positive strengths and behaviors
Identify and maintain current information concerning community resources (housing, food, banks, heat assistance, etc.) and application procedures for accessing public assistance
Complete reports as directed
Monitor client medications, order refills, act as liaison to medical provider
Dispense medication per orders to clients
Assist individuals in making and keeping necessary appointments (physician, mental health,
Participate in treatment team meetings
Transport clients as needed.
Report all verified or suspected occurrences of child abuse and neglect
Qualifications
Minimum 2 years’ working with individuals in recovery; residential experience helpful
De-escalation and/or Crisis Management training
Basic computer skills with Office 365 products; working knowledge of relational databases or electronic health records systems helpful
Education/Licensure/Certification requirement
High School diploma or equivalent
PSS, THW including Peer Mental Health Certifications or to obtain within 6 months.
