Care Manager RN

Full‑Time / Day Shift. This position includes working on some weekends as needed.
Description
The RN Case Manager is an expert professional registered nurse who is responsible for developing and managing patient care outcomes for an entire patient case load. The RN Case Manager may perform assessment, treatment or care for patients of all ages, including neonate, pediatric, adolescent, adult and geriatric, based on population focus. Duties also include complex discharge planning and utilization review.

Required Qualifications
Oregon Registered Nurse License upon hire.
3 years Acute care hospital experience.
Preferred Qualifications
Bachelor's Degree in Nursing.
1 year preferred experience in home health, mental health, substance use, hospice, and/or utilization review.
Preferred progressive nursing leadership experience, including charge nurse role.
Compensation & Benefits
Base pay: $45.99 – $71.40 per hour. Additional compensation may include shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
