Main duties of the job Your role and responsibilities
As a Care Coordinator, your mission will be to support people in living well and independently at home. Youll play a key part in transforming how care is delivered - by building strong, trusting relationships, streamlining services, and helping to reduce unnecessary hospital admissions. Youll ensure care is delivered closer to where people live, making it more accessible, personalised, and effective.

Job responsibilities What we are looking for

Confidence using digital tools and a willingness to learn.
Experience in health, social care, or community settings is helpful - but transferable skills from other sectors are welcome.

Person Specification Qualifications Relevant qualification such as NVQ4 or equivalent level of knowledge and experience.
