Overview Inpatient Case Manager, MSW is active in the Sentara Regional Resource Pool located in the Hampton Roads area. The position supports Days/PRN shifts with flexibility across four hospitals in the Southside or Western Tidewater regions. Location and unit assignment are based on greatest need and must be within 75 miles and 90 minutes of all four hospitals in the assigned region. Operational hours are 8:00 AM – 4:30 PM, seven days per week. Prescheduled hours require at least 48 hours per calendar month, with additional weekend availability as applicable.

There is a high degree of flexibility for hours and shifts, with regional/local travel and incentive opportunities. This role involves regional coverage across hospitals and units within the case management specialty.
Responsibilities As an Inpatient Care Manager, provide medical social work services to hospital patients. Assess social, psychological, cultural, environmental, and financial situations, as well as disposition needs. Collaborate with patients, families, healthcare team members, and community organizations; advocate to identify needs and resources in hospital and community settings. Develop and evaluate transition plans considering diagnosis, prognosis, treatments, current and past services, goals, provider options, and available health care benefits for continuity of care. Work in hospital settings as part of the inpatient case management team.
Be flexible to work across facilities within the assigned region and participate in shift and unit assignments as needed to meet patient and organizational needs.
Qualifications Education: Master's degree in Social Work
Certification/Licensure: Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility; BLS required within 90 days of hire
Experience: One year of related experience
Geographic requirement: Within 75 miles and 90 minutes of all four hospitals in the assigned region
