Your Role: Hospice RN Case Manager

As a Hospice RN Case Manager, you'll provide one-on-one care to patients with life-limiting illnesses, helping them live with dignity, comfort, and peace. You'll lead the care team, coordinate services, and support patients and families through every step of their journey.

What You'll Do

• Develop and manage individualized care plans
• Provide symptom management and emotional support
• Educate patients and families on end-of-life care
• Coordinate with physicians and interdisciplinary teams
• Supervise Hospice Aides and LPNs
• Document care accurately using point-of-care technology
• Participate in IDT/IDG meetings and case conferences
• Ensure timely communication and care coordination

What You'll Need

• Graduate of an accredited nursing program
• Active RN license in the State of Iowa
• Minimum 1 year of professional nursing experience (acute care preferred)
• Hospice or home care experience preferred
• Strong communication and interpersonal skills
• Valid driver's license and reliable transportation
