As a Hospice RN Case Manager , youll provide one-on-one care to patients with life-limiting illnesses, helping them live with dignity, comfort, and peace. Youll lead the care team, coordinate services, and support patients and families through every step of their journey.
What Youll Do
Develop and manage individualized care plans
Provide symptom management and emotional support
Educate patients and families on end-of-life care
Coordinate with physicians and interdisciplinary teams
Supervise Hospice Aides and LPNs
Document care accurately using point-of-care technology
Participate in IDT/IDG meetings and case conferences
Ensure timely communication and care coordination
What Youll Need Graduate of an accredited nursing program
Active RN license in the State of Iowa
Minimum 1 year of professional nursing experience (acute care preferred)
Hospice or home care experience preferred
Strong communication and interpersonal skills
Valid drivers license and reliable transportation
