Overview
We are hiring a full-time RN Case Manager to join the Athena Home Health & Hospice team in Fairfield and New Haven Counties!

This is an on-the-road position. Weekly pay!

As an RN Hospice Case Manager, you plan, organize, and direct hospice care. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Experience & Education

Graduate of an accredited school of nursing; bachelor’s degree preferred.

One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.

Must possess an active and unencumbered RN license in Connecticut.

CPR certification.

Duties & Responsibilities

Supervise hospice aides.

Complete an initial, comprehensive assessment of the patient and family to determine hospice needs.

Provide a complete physical assessment and history of current and previous illnesses.

Provide professional nursing care by utilizing all elements of the nursing process.

Assess and evaluate the patient’s status by writing and initiating a plan of care, regularly re-evaluating needs, and revising the plan of care as necessary.

Initiate the plan of care and make necessary revisions as patient status and needs change.

Use health assessment data to determine nursing diagnosis.

Develop a care plan with goals based on nursing diagnosis, incorporating palliative nursing actions, and involve the patient and family in planning.

Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician in the plan of care.

Counsel the patient and family in meeting nursing and related needs.

Provide health care instructions to the patient as appropriate per assessment and plan.

Assist the patient with activities of daily living and support self-sufficiency and comfort care where appropriate.

Act as Case Manager when assigned and coordinate care for the caseload.
