Overview Hospice Registered Nurse (RN) Case Manager (RNCM).

Schedule: Monday through Friday 8:00 am to 4:30 pm
Company: Redcrest Hospice
What You Will Do Symptom management: Through thorough assessment, you will work with the patient’s attending physician and the hospice Medical Director to ensure symptoms are well managed.
Plan of Care: Evaluate the patient’s treatment and medical condition according to the plan of care. Individualize the plan of care and adjust as needs change.
Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with admissions, transfers, and discharges.
Family Educator/Advocate: Counsel patient and family on disease progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
Documentation: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.
Qualifications Registered Nurse with current license in the state of employment.
Minimum one-year experience as an RN in an acute care setting is preferred.
Hospice experience is desired.
Valid driver’s license and auto insurance in your name as a driver.
Capable of all physical demands.
