$23.87/hr - $30.31/hr

Position Summary:
We’re looking for a compassionate, respectful, full-time Healthcare Case Manager for our Jackson Heights location . The successful candidate will ensure the quality care of the Apicha CHC’s patients by promoting quality and cost-effective outcomes. Provide case management services to children, families, and adults to ensure the services they receive are appropriate for their specific needs. A Healthcare Case Manager also works with the patient’s support systems to make sure care arrangements align with the patients' needs, with input from the multidisciplinary Care Team.

Job Responsibilities:

Client Services and Direct Care: Provide proactive primary case management including intake, assessments, and care plan development to ensure access to medical care and other services, maintaining communication with Apicha’s multidisciplinary care team to promote positive health outcomes.

Collaboration and Services: Work collaboratively with patients and their families to identify needs and coordinate with external providers to ensure comprehensive care.

Compliance: Maintain up-to-date, organized patient records, and complete all necessary documentation and reports in a timely manner.

Qualifications:

Bachelor’s degree in a related field (e.g., health, nursing, mental health, child and family studies, public health, or other human services) with 2 years of relevant experience; or a bachelor’s in a non-related field with 4 years of relevant experience.

Experience working with target populations such as children and families, individuals with Serious Mental Illness, disabilities, SUDs, LGBTQ+ communities, and those living with HIV/AIDS or hepatitis is preferred.

UAS-NY certification required within 1 month of hire.

Fluency in Spanish or an Asian language is preferred.

Excellent communication skills in English are required.

Ability to work effectively both independently and collaboratively.

Proficiency with various applications (Outlook, Word, Excel, PowerPoint, eCW, eICARE, AZARA, HCS, MAPP, Healthix, PSYCKES, etc.) is required.

Strong customer service skills to address patient and provider concerns.

Flexibility to work some evenings and weekends.

Ability to multitask and manage multiple projects in a stressful environment.
