Position Summary
We are seeking a compassionate, respectful full-time Healthcare Case Manager for our Jackson Heights location. The successful candidate will ensure quality care for patients by promoting positive, cost-effective outcomes, providing case management services to children, families, and adults, and working with support systems and the multidisciplinary Care Team to meet patient needs.
Job Responsibilities
Client Services and Direct Care: Provide proactive primary case management, including intake, assessments, and care plan development to ensure access to medical care and other services, with regular communication to ensure positive health outcomes.
Collaboration and Services: Collaborate with patients, families, and external providers to develop care plans tailored to specific needs.
Compliance: Maintain accurate, organized, real-time patient records, and complete all necessary documentation promptly.
Qualifications
Bachelor’s degree in a related field with 2 years of relevant experience, or a non-related degree with 4 years of relevant experience.
Experience with target populations such as children, families, individuals with SMI, disabilities, SUDs, LGBTQ+, or those living with HIV/AIDS, hepatitis, or immigrants is preferred.
UAS-NY certification required within 1 month of hire.
Fluency in Spanish or Asian languages preferred.
Excellent communication skills in English.
Ability to work collaboratively and independently.
Proficiency with relevant applications (Outlook, Word, Excel, etc.).
Strong customer service skills and flexibility to work evenings and weekends.
Ability to multitask in a stressful environment.
