Position Function
The clinical case manager will support and serve youth and their families by providing and coordinating case management services, therapeutic services, crisis management, and transitional services. They will respond to referrals and coordinate admissions for assigned programs, provide admission assessments, develop service plans, assist in setting goals, and coordinate resources to stabilize behaviors and provide independent living skills for youth transitioning to permanence or independence. The job requires sensitivity to the service population’s cultural and socioeconomic characteristics.

Education Requirements
A Master’s degree in social work, psychology, or a related human services field from an accredited college or university is required; additional certification is preferred.

Experience Requirements
The position requires two years of cumulative experience with an undergraduate degree that includes working with youth, crisis intervention, intake and assessment, and case management. Understanding of crisis intervention and trauma‑informed care interventions, counseling techniques with adolescents and families, good verbal and written communication skills, and basic computer skills is essential.

Functional Requirements
Requirements include providing expedient and ongoing assessment, service planning, case management, and transition planning for all assigned clients. Maintain accurate and timely documentation of services for all assigned clients. Prepare and submit all required documentation, reports, and statistical data in a timely manner. Follow policies and procedures of ACH and represent the agency in a positive and professional manner. Coordinate collaboration with referral agencies, family members, and other support systems to meet the needs of youth. Effectively communicate with campus staff to coordinate service plans and lead groups (life skills, team building, etc.) with youth in assigned programs. Residential programs operate 24/7. This position provides on‑call support for clinical and program staff.

Key Expectations/Responsibilities

Provide 24‑hour response to all client referrals in coordination with program leadership.

Manage admissions to assigned programs through screening and assessment of all referrals and facilitation of placements.

Facilitate and document admission assessment and service planning for all assigned clients.

Complete all admission paperwork within required time frames.

Provide expedient case management and therapeutic interventions.

Provide skills training for assigned clients.

Implement group programming to address the social and emotional needs of clients related to achievement of individual goals.

Provide case management services; coordinate with TDFPS caseworkers, families, community, and mainstream support resources.

Document and assess progress of each assigned client.

Develop discharge and aftercare plans and follow‑up referrals.

Provide aftercare services as required.

Community Relations

Coordinate the collaborative relationships and services to referral agencies.

Build and maintain positive working relationships with local human and youth services agencies.

Represent ACH in a professional and positive manner.

Documentation and Reporting

Maintain up‑to‑date case records.

Develop and document initial assessment and initial plan of service.

Provide regular case management services for each assigned client.

Document case progress notes.

Communicate and coordinate client needs.

Communicate goals and service plan with the direct care team.

Assist in development of program‑related forms and measurement tools.

Develop, review, and update service plans.

Complete a discharge and aftercare plan for each client.

Assist with fundraising or other agency functions as needed.

Other applicable duties as assigned by supervisor.
