Overview Client Care Coordinator-Scheduler role at Avalon Essential Home Care. This position focuses on delivering a positive and satisfying experience for clients, caregivers, vendors, and future candidates.
Base pay range $45,000.00/yr - $55,000.00/yr
Responsibilities Reflect the core values of AE Home Care.
Schedule clients and caregivers in the company software.
Meet with clients and family members to discuss their needs and provide solutions in the form of a service/care plan.
Coordinate various aspects of a client’s care with other team members.
Introduce clients and caregivers to new clients and new caregivers.
Maintain client and responsible party records documenting all SUP Visits.
Recognize and pursue opportunities to modify service plans to best support ongoing needs of clients with approval from the Clinical Supervisor.
Demonstrate open and effective communication with clients, family members, colleagues, caregivers, referral providers, care providers, and the franchise owner.
Work with clients and their families on issues to ensure resolution and delivery of needed services.
Maintain regular attendance at the office to execute job responsibilities.
Adhere to all company policies, procedures and business ethics codes and ensure they are communicated and implemented within the team.
Rotate Emergency On-Call with the Senior Client Care Coordinator.
Participate in caregiver meetings as needed.
Perform other functions as deemed necessary.

Work Environment This job operates in a professional office environment and routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Work Hours This is a full-time position. Days and hours are Monday through Friday, 8:30 a.m. to 4:30 p.m. Occasional evenings and weekends may be required, with a rotation of Emergency On-Call with the Senior Client Care Coordinator.
