Become part of the Hope At Home Family and apply TODAY (serious applicants only)! We currently have a position for an experienced Home Health Care Registered Nurse, with preferred experience in Mediport, PICC Line, Wound Care, and Infusion.

The corporate office is located in Southfield, MI, and the individual will provide RN support throughout the Tri-County area (Oakland, Macomb, Wayne).

Responsibilities:

Provide services according to the plan of care.

Conduct initial and ongoing patient evaluations.

Develop and revise care plans.

Administer skilled nursing care and preventive procedures.

Document patient visits and care summaries in a timely manner.

Coordinate services and inform staff of patient condition changes.

Counsel patients and families on nursing needs.

Participate in in-service programs and adhere to agency policies.

Process orders, notify physicians, and complete certifications and discharge summaries.

Refer patients to specialized therapists as needed.

Supervise and teach other nursing personnel.

Participate in patient care conferences, peer review, and performance improvement.

Take on-call duties nights, weekends, and holidays as assigned.

Complete assessments, transfers, and discharges within specified time frames.

Utilize ICD-10 codes appropriately.

Working Environment:
Work indoors at the agency office and in patients' homes, with travel within the service area.

Qualifications:

Graduate of an approved nursing program and current Michigan license.

One year of nursing experience.

Kinnser EMR experience preferred.

Home healthcare experience preferred.

Experience with Wound Vac, PICC line, and Mediport care preferred.

Pediatric experience is a plus.
