Client Care Coordinator (LMSW or LMHC)
Location: New York, NY

Base pay range: $54,999.85/yr – $54,999.85/yr ($54-57K)

Responsibilities

Conduct initial psychosocial history interviews on all clients during the Admission/Orientation phase.

Determine mental health status during Admission/Orientation and at appropriate times during Treatment Phases.

Coordinate all mental health treatment and referrals for clients in all phases.

Consult with primary counselors on clients with special needs.

Provide supervision or treatment of all cases requiring social worker level involvement by managed care or other referral agencies.

Attend seminars and training on pertinent issues.

Develop and maintain referral linkages with outside agencies and organizations to meet the needs of program clients.

Participate on interdisciplinary treatment teams and in case conferences.

Maintain up-to-date case notes for clients on caseload.

Work in close liaison with program Nurse Practitioner-Psychiatrist.

Education and License

Master of Social Work from an accredited institution.

NY State Licensed LMSW or LMHC.

Qualifications

2+ years of post‑graduate social work experience, preferably with chemically dependent and/or mentally ill individuals.

Experience delivering social services in a DHS‑regulated shelter environment.

Knowledge of DHS policies, procedures, and protocols.

Diplomacy and discretion in dealing with sensitive issues.

Business acumen to exercise good judgment and decision‑making.

Proficiency with Microsoft Office.

Bilingual language skills a plus.
