What You Will Do
Perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record. This may begin as an Independent 1099 Contractor position with potential to transition to full-time W2 employment with employee benefits.
Abide by the plan of care and orders of the practice; provide prevention and intervention for multiple disease conditions through motivational coaching.
Develop positive interactions with patients on behalf of our practices and work to improve revenue by creating billable CCM episodes and managing chronic condition care.
Develop detailed care plans for doctors and patients focused on prevention and intervention; understand health care goals in chronic disease management.
Attend regularly scheduled meetings (e.g., bi-monthly staff meetings, monthly one-on-ones) as part of the scope of work.
Maintain HIPAA privacy compliance while performing duties from a home office.
What We Are Looking For
Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
Current COMPACT license to practice as an RN/LVN/LPN in the residence state with no disciplinary actions noted.
A minimum of two (2) years of clinical experience in Med/Surg, Case Management, and/or home health care.
Hands-on experience with Electronic Medical Records and proficiency with Windows desktop and applications (Microsoft Office 365, Teams, Excel), in a HIPAA-compliant home environment.
Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
Proficiency with various computer programs and a willingness to learn required technologies.
High-speed internet and a desktop or laptop computer (Windows or Mac; Chromebooks not accepted).
Excellent verbal, written, and listening skills.
