Overview
The position is under the direction of the Program Manager of Child and Adolescent Case Managers and, within federal, state and local guidelines, performs routine case management work for young children and adolescents. The role involves developing and maintaining long-term contact with individuals, their families, and service providers for medical, social, educational, and related service needs. This position oversees the care of children and adolescents in Behavioral Health services assigned to various Levels of Care (LOC) and works to ensure continuity of care for individuals on their caseload. Works under moderate supervision, with limited latitude for initiative and independent judgment.

Education, Training, And Experience
Graduation from an accredited four-year college or university with major coursework in a human service field as required to qualify as a Qualified Mental Health Professional (QMHP) and six (6) months prior experience in a mental health field preferred.

Registration, Certification, Licensure, And Other Qualifications

Individual needs to meet the requirements to qualify as a QMHP (Qualified Mental Health Professional).

All job specific training is required to be completed within 30 days of hire.

This position requires a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Center's insurance requirements.

Knowledge, Skills, And Abilities

Understanding and practice of Trauma Informed Care principles.

Knowledge of community resources, case management delivery systems, and program policies and procedures.

Ability to assess individuals' needs, coordinate services, and communicate effectively.

May train others.

Ability to work independently and make decisions but seeks guidance when needed.

Ability to make decisions based on education and/or clinical experience, consistent with the Center's Guiding Principles, Policies and Procedures, and Standards of Care.

Understanding of community resources available for Denton County and willingness to acquire new resources in the community.

Ability to work with clients from diverse cultural backgrounds, sexual orientations, and belief systems.

Ability to work with clients who may display symptoms of mental illness, including risk factors such as aggression, poor hygiene, or suicidal/homicidal ideation.

Essential Duties

Attend and participate in scheduled team meetings and sign in as required.

Provide courteous, professional services at all times and maintain professional boundaries.

Interact with Center staff, individuals, and community members in a professional manner.

Provide case management and skills training as appropriate for individuals approved for services under Texas Resilience Recovery (TRR) guidelines.

Provide resources/referrals to meet individual needs and help navigate different systems.

Support teammates by providing services to individuals on caseload as needed.

Complete community-based visits as required, based on LOC.

Ensure services are provided as required by level of care assignments each month and maintain engagement and contact standards per TRR guidelines.

Administer clinical assessments and paperwork to determine appropriateness of level of care, including CANS, Recovery Plan, Rights Review, and consent forms.

Maintain charts in compliance with HHSC contract requirements and Center policies; respond to chart audits by assigned deadlines.

All documentation to be completed within Center policies and procedures and signed in black ink with credentials and date.

Provide emergency/crisis intervention when necessary per HHSC and Center policies and procedures.

Participate in the Center's Zero Suicide Initiative and coordinate with relevant programs (e.g., MCOT, Crisis Prevention) for crisis intervention.

Collaborate with the team to review crisis incidents and ensure HHSC guidelines and Center policies are followed.

Other duties as assigned; complete all assignments in a timely manner.

Full Time: Non-Exempt

Work Model: In Person

Schedule: Monday through Friday, 8:00 AM–5:00 PM

Pay Range: $19–$20 per hour
