Overview Service Coordination, Inc. is looking for Supports Planners who are passionate about serving their community and advocating for underserved individuals. This role focuses on providing case management and supports planning services to people with disabilities, behavioral challenges, medically complex needs, transitioning youth, and other populations in Maryland.
Responsibilities Provide targeted case management by advocating, arranging, and monitoring services for people with Medicaid Home and Community Based Services
Support planning by identifying resources, coordinating services, and ensuring outcomes documented in Person Centered Plans or POS
Engage in person-centered planning with people served; develop POSs in collaboration with individuals and their supports
Research, identify, refer, and coordinate resources to achieve POS outcomes; share resources with the team
Monitor POS and conduct follow-up activities; perform on-site visits and timely communication with providers
Ensure services meet state mandated standards; maintain HIPAA confidentiality and secure records
Adhere to SCI policies and procedures; maintain performance metrics and time management tools
Position Requirements Bachelor’s or Master’s degree in one of the following: Psychology, Sociology, Social Work, Nursing, Counseling, Human Services Administration
Valid Driver’s License and insured vehicle

Experience working with people with disabilities is preferred; experience with person-centered planning is preferred
Physical Demands Generally, standard office environment. Ability to operate a computer and mobile devices; occasional light lifting may be required. May need to drive and travel as part of the role.
