Position Summary: Provide Case Management services and support to people who have a disability and/or medical need that require assistance with activities of daily living. Primary objectives include establishing positive working relationships, exploring options, establishing Person‑Centered Plans of Service, meeting state mandate requirements, identifying supports, and ensuring service quality.

Position Requirements

Bachelor's or master's degree in one of the following concentrations:

Psychology

Sociology

Social Work

Nursing

Counseling

Human Services Administration

Valid driver’s license and insured vehicle.

Experience

Experience working with people with disabilities (preferred).

Experience with person‑centered planning (preferred).

Essential Duties

Advocate on behalf of the people served; provide resource information and education for self‑determination.

Collect information and documentation related to eligibility for Supports Planning services.

Plan and facilitate the development and updates of Person‑Centered Plans of Service.

Research, identify, refer and coordinate resources and services to achieve outcomes specified in the person’s POS.

Monitor POS and conduct follow‑up activities, including on‑site visits.

Adhere to SCI policies and procedures, and maintain HIPAA records in secure locations.

General office environment. Must remain stationary >95% of the time. Operate computer and related office equipment. Occasionally move office equipment weighing 10-25 pounds. Allow required driving. Provide verbal communication as needed. Reasonable accommodations may be made.
