Calling all Morgan State Bears!! – Case Manager (Supports Planner)
Service Coordination Inc. (SCI) is seeking passionate Supports Planners who are eager to serve and advocate for underserved individuals in our community. As a Supports Planner, you will provide targeted case management and person‑centered planning to help individuals with disabilities, medical needs, and special populations achieve independence and quality of life.

Benefits & Compensation

Competitive base salary range of $64,395‑$79,861 for all incoming Case Managers, based on education, experience, and certifications.

Quarterly bonus potential and full reimbursement for travel expenses.

Key Responsibilities

Advocate on behalf of people served, providing resource information and education on self‑determination.

Collect and document eligibility information for supports planning services.

Plan, facilitate, develop, and continually update Person‑Centered Plans of Service (POS) and POS tools.

Research, identify, refer, and coordinate services and resources to achieve POS outcomes; complete required paperwork and follow‑up for waivers.

Conduct regular visits, monitor progress, and complete Reportable Events (RE) as required by regulations.

Maintain compliance with SCI policies, HIPAA confidentiality, and tracking/time‑management tools.

Participate in team meetings, provide constructive feedback, and contribute to a culture of diversity and inclusion.

Position Requirements & Qualifications

Education: Bachelor’s or Master’s degree in Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling, or related field.

Valid driver’s license and insured vehicle; mileage reimbursement at federal rate.

Experience preferred with people with disabilities and person‑centered planning.

Knowledge of state‑mandated supports planning standards and regulations.

Strong problem‑solving, decision‑making, interpersonal communication, teamwork, and professionalism.

Minimum Qualifications

Bachelor’s degree (or higher) in Human Services, Psychology, Sociology, Social Work, Nursing, or Counseling.

Valid driver’s license and insured vehicle.

Physical Demands

Primarily office‑based; must remain stationary >95% of the time.

Must be able to operate computer equipment; may occasionally lift or move office equipment weighing 10‑25 pounds.

Vision: close vision required.

Must be able to operate a motor vehicle.
