Responsibilities
Advocate on behalf of individuals, provide resource information, and educate them to become more self-determined.
Provide education to individuals and their families and on how to coordinate and advocate for services.
Encourage individuals to empower themselves and achieve independence; develop and monitor Person-Centered Plans (PCPs) and ensure services meet state-mandated standards.
Collect information for eligibility for DDA services, complete Comprehensive Assessments, and coordinate PCPs with individuals and their support networks.
Travel to meet with support staff and provide information, referrals, and assistance; assist individuals with transitions and waivers; coordinate with providers for community integration.
Monitor PCPs, conduct on-site visits, follow regulations, and prepare required documentation (e.g., RECON, AIR) for program eligibility and ongoing services.
Communicate with providers to resolve concerns; oversee documentation for medical assistance applications; ensure HIPAA compliance and secure records.
Qualifications
Education: A bachelor’s degree in a human services field from an accredited program; or a bachelor’s in a non-human services field with two years of experience in a human services field.
Experience: Experience working with people with developmental disabilities is preferred.
Required competencies include collaboration, effective communication, customer focus, interpersonal savvy, trust, tech savviness, accountability, valuing differences, resourcefulness, adaptability, and planning and prioritization.
Education Requirements
As above: bachelor’s in human services or bachelor’s in another field with relevant human services experience.
Benefits and Compensation
Competitive base salary range: $64,395–$79,861, determined by education, case management experience, and relevant certifications.
