We are searching for a Case Manager to join our team of healthcare professionals. The Case Manager is an individual with hands‑on experience and knowledge regarding the nature and treatment of alcoholism and drug addiction. The Case Manager acts as a client advocate and assists clients in problem‑solving obstacles, providing intervention and support to clients when appropriate. The Case Manager will maintain accurate and up‑to‑date documentation in the electronic medical record, advocate with primary care providers to ensure access to quality care, actively participate in multidisciplinary meetings, and create and maintain positive relationships with the clinical staff to ensure appropriate and timely care for clients.
Responsibilities
Completes Intake and Case Management Assessments.
Coordinates inter‑departmentally to facilitate clients going to approved medical appointments.
Begin correspondence with any legal contacts: probation, courts, and Judicial assistants to allow clients to make informed decisions regarding discharge planning.
Attempt to prevent any further legal stress or charges while in treatment.
Link clients with community resources.
Based on clinical recommendations and approval, set up appropriate aftercare plans specific to the client’s needs. Coordinating with referral sources, PC/IOP programs, psychiatrists, and individual therapists to convey all appropriate information.
Attends regular clinical staff meetings.
Demonstrates adaptability and flexibility to changes in the work environment.
Perform other duties as requested.
Qualifications
Verbal and written communication skills required, must be able to speak clearly and concisely over the telephone.
Previous treatment experience required.
Ability to work independently and to adapt to a fast changing environment.
Ability to multitask in a high pressured environment.
Ability to be compassionate to clients.
Ability to utilize strong ethical boundaries, when needed.
Excellent time management skills.
Familiarity with electronic medical records systems, KIPU preferred.
