Overview RN Care Manager - Clinical Resource Management at Kadlec Regional Medical Center, Richland, WA. Under the direction of the Director, provides clinically-based case management to support the delivery of efficient patient care. Has the overall accountability for the integrated responsibilities of case management; utilization management, discharge planning, facilitation and coordination of care. Collaborates with other members of the health care team to identify appropriate utilization of resources and to ensure reimbursement. Utilizes criteria to confirm medical necessity for admission and continued stay. In conjunction with the patient, family and healthcare team, creates a discharge plan appropriate to the patient’s needs and resources.
Responsibilities Provide clinically-based case management to support the delivery of efficient patient care.
Manage utilization, discharge planning, facilitation and coordination of care.
Collaborate with the health care team to identify appropriate utilization of resources and to ensure reimbursement.
Utilize criteria to confirm medical necessity for admission and continued stay.
Develop discharge plans with patients, families and the health care team.
Contribute to the Kadlec/Providence culture of patient-focused, whole-person care.
Shift Details Full time, Day Shift, 40 working hours weekly
Required Qualifications Graduation from an accredited school of nursing.
Washington Registered Nurse License at hire.
Preferred Qualifications Bachelor’s Degree from an accredited educational institution.
2 years of recent inpatient clinical experience in an acute care facility.
Previous Case Management, discharge planning or utilization review experience.

The base pay range is listed; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
