Position Summary As a Case Manager I , you will work collaboratively with case managers, supervisors, integrated health team members, and community-based partners to promote long-term solutions for families, children, and individuals experiencing homelessness or who have formerly experienced homelessness. You will develop and implement care plans, coordinate services and resources, monitor client progress, provide advocacy and support, facilitate referrals to other services, and maintain case documentation and data collection. Services are grounded in motivational interviewing, stages of change, strengths-based case management, housing first, harm reduction, cultural responsiveness, and trauma-informed care. This role focuses on delivering outreach services, facilitating referrals and linkages to mainstream services and housing, and providing crisis intervention.
Note: The selected candidate will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing timeline depends on submission of documents necessary for clearance. A start date will be scheduled once credentialing and privileging are complete.

Essential Job Functions Work as a member of the CCH Community Resources Team to provide face-to-face services to homeless individuals, including meetings outside the office when needed. Assist clients in recognizing and defining their service needs, inform them of available services, and help them access goods, services, financial benefits, IDs, housing, and other needs.
Collaborate with homeless service providers in the community and CCH programs to ensure coordination, communication, and continuity of services.
Provide services to clients in a trauma-informed, culturally competent manner.
Advocate for individual clients and, when needed, for systemic change to benefit people experiencing homelessness.
Build relationships with clients to support housing, benefits, and other resources needed to end homelessness.
Transport clients using a CCH vehicle or approved personal vehicle.
Accept referrals from CCH staff and other agencies and collaborate with staff of all agencies to provide high-quality service.
Meet with clients through walk-in or scheduled appointments as needed.
Complete all documentation and data entry in HMIS and EHR in a timely, thorough manner and in compliance with HIPAA regulations.
Record all services in the EHR and maintain up-to-date client records.
Record telephone and other communications as needed in the EHR.
Manage tasks and scanning to maintain a complete and current chart.
Maintain accurate information regarding client insurance status.
Utilize the EHR and future upgrades as directed.
Assess client needs and develop short- and long-term service and treatment plans.
Coordinate ongoing clinical and case management services for individuals.
Respond to and manage work-related emails and voicemails.
Qualifications Summary High School Diploma or GED required. Bachelor’s degree in psychology, Social Work, Human Services, or Criminal Justice preferred.
A combination of education and experience may be substituted on a case-by-case basis.
1–2 years of experience working with diverse populations facing mental health challenges, with preference for those diagnosed with serious and persistent mental illness (SPMI).
Experience with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred.
Strong understanding of housing first principles, trauma-informed care, and harm reduction strategies.
Excellent interpersonal and communication skills; ability to build trusting relationships with residents.
Ability to manage a diverse caseload, prioritize tasks, and work independently while collaborating with a team.
Proficiency in Microsoft Office Suite.
Knowledge of local housing resources, benefits programs, and social service agencies preferred.
Bilingual or multilingual skills preferred.
Must have a valid Colorado Driver’s License.
Compensation : $20.64 – $25 per hour. Place in compensation range depends on total relevant years of experience.
