Case Manager- QMHP Crisis Care Counselor Mobile

Schedule – Specific Hours: Wed, Thurs, Sun, & Every other Friday. 6:00 am – 6:00 pm.

Responsibilities

Provide crisis intervention, de‑escalation, emotional support, and suicide risk assessment to callers via AID’s 24/7 crisis line.

Manage and assess incoming Mobile Crisis referrals from 988, local police, and community services through the crisis line.

Deliver in‑person Mobile Crisis Response throughout Kendall and Southern Kane County for individuals unable to be de‑escalated over the phone.

Conduct in‑person assessments for suicide risk, level of care, and support access; collaborate on safety plans with clients and collateral contacts.

Collaborate with AID staff and external service providers to meet client needs and promote community stabilization.

Maintain documentation required for crisis intervention services on phone and in the community.

Supervise MHP staff and Recovery Support Specialists during community crisis interventions and approve assessments.

Attend scheduled work hours, remain flexible, and respond to calls from shift start to end; provide crisis line coverage when not on mobile response.

Consult with family, case managers, existing service providers, and external resources as appropriate, using intervention tactics to reduce symptomology.

Provide independent program coverage when scheduled or in emergency situations.

Complete clinical assessments meeting program funder and administrative requirements; provide effective telephone counseling through active listening and motivational interviewing.

Submit required documentation in a timely manner (billing, daily logs, dispatch logs, 988 follow‑up, and iCarol logs) before shift end.

Attend clinical supervision at least once per month and as required.

Meet monthly productivity standards when applicable.

Document crisis intervention and suicide assessments per Rule 132 standards and complete required billing notes.

Develop crisis safety plans and resource follow‑up, collaborating with all parties per Rule 132, payer, and accreditation standards.

Provide case management follow‑up to support client linkage to services.

Maintain records on the MCR‑T, assist the MCR‑T Lead in quality assurance of records.

Communicate effectively with agency staff, providers, families, and others to address client needs.

Navigate Cx360, opening client profiles, attaching necessary paperwork, and uploading encounters and notes.

Manage crisis line documentation service and submit required documentation.

Identify emergency situations, coordinate with Manager and Director per procedure, and complete incident reports, petitions, and case notes accurately.

Participate in training and development opportunities as assigned.

Promote mental health awareness internally and externally.

Adhere to all Program 590 guidelines and expectations outlined by the 590 Mobile Crisis Response Grant and IDPH Department of Mental Health.

Education & Qualifications
Master’s degree in a human services field (QMHP) required. At least one year of experience working with individuals with mental illness. Knowledge of DSM V‑TR or current edition and Rule 132. Bilingual‑Spanish preferred.

Experience: At least one year in behavioral health preferred.

Physical, Equipment & Additional Requirements

Ability to communicate in person, telephone, and group settings; frequent standing, walking, navigating stairs, and traveling to client homes and community locations.

Must be able to drive and work overnight shifts.

Position operates in both a professional office and off‑site community settings; uses standard office equipment (computers, phones, copiers, filing cabinets).

Flexibility to work evenings, overnight shifts, weekends, rotating holidays, and on‑call rotations.

Cover non‑scheduled shifts as needed.

Reliable transportation with valid automobile liability insurance per AID policies.

Valid Illinois driver’s license; ability to drive self and clients in agency or personal vehicles.

Cultural competency and community awareness while working with diverse populations.

Potential requirement to work at multiple locations.

Complete required training (First Aid, CPR, Non‑violent Crisis Intervention, CEU’s, and other assignments) within 30 days of hire.
