Residential Case Manager - Community Residential Rehabilitation (CRR) Level 3 Program

Claratel Behavioral Health is seeking a compassionate and organized Residential Case Manager to deliver direct, semi-independent support and essential skills training to adults with behavioral health challenges in our Community Residential Rehabilitation (CRR) Level 3 program, in compliance with Georgia DBHDD regulations.

Job Summary

The Residential Case Manager provides direct support services to residents to assist them in achieving the goals outlined in their treatment plans. This role focuses on promoting recovery, teaching independent living skills, and facilitating community integration within the context of a semi-independent, transitional residential setting.

Essential Duties and Responsibilities
Direct Client Services and Skills Training

Provide a minimum of 3 hours per week of dedicated skills training, community integration activities, and/or personal services to each assigned resident as stipulated in the treatment plan.

Deliver services to assist residents with the development and mastery of independent living skills, including:

Daily Living Skills: Meal planning and preparation, grocery shopping, household management (cleaning, laundry), and personal hygiene.

Financial Literacy: Budgeting, managing finances, and accessing entitlements.

Community Integration: Utilizing public transportation, accessing community resources (e.g., library, social groups), and engaging in leisure activities.

Health and Wellness: Appointment scheduling, medication monitoring adherence, and general health management.

Ensure all support provided is accurately and timely documented in the resident's clinical record, including service logs and progress notes, adhering to Claratel Behavioral Health’s documentation standards.

Facilitate access to necessary external services, such as vocational rehabilitation, primary healthcare, and ongoing mental health treatment.

Required Qualifications

Education: H.S. Diploma or GED.

Experience:

Minimum of one (1) year of experience in a behavioral health or substance use disorder setting. Residential experience preferred.

Knowledge/Skills:

In-depth knowledge of person-centered planning and psychosocial rehabilitation principles.

Strong ability to build rapport, set boundaries, and motivate individuals toward recovery and independence.

Excellent written and verbal communication skills for documentation and professional interaction.

Other:

Must be able to pass a satisfactory criminal history background check.

Valid Georgia Driver’s License and insurable driving record.

Working Conditions

This is a part-time position requiring flexibility, including the ability to work evenings, weekends. The position requires working in a community-based residential setting.

Pre-employment drug screening and successful completion of a criminal history background check is required.
