Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Pay Rate: $23.50/hour for Case Manager, $25.75/hour for QMHP
Responsibilities include: Developing and implementing individual treatment plans using IM+CANS to enhance clients' community living skills.
Completing documentation of mental health services per state and agency standards.
Collaborating with a team of behavioral health providers and outside agencies to ensure client-centered care.
Providing some remote work options, subject to performance and operational needs.
Additional Job Responsibilities: Providing flexible scheduling and coverage as needed.
Ensuring compliance with all regulations and policies.
Communicating with clients, families, and community partners effectively.
Maintaining professionalism and proper boundaries.
Meeting service hour and documentation standards.
Assisting clients with daily living activities, budgeting, and benefits management.
Responding to emergencies and documenting incidents accurately.
Facilitating client and family engagement and wellness coaching.

Qualifications: Bachelor’s degree in human services or related field (required); Master’s degree for QMHP role.
Experience in social services preferred.
Valid Illinois driver’s license and auto insurance.
Physical and Equipment Requirements: Ability to navigate stairs and provide in-home services.
Proficiency with computers, copiers, phones, and basic household appliances.
Additional Requirements: Valid driver’s license and insurance.
Certifications in First Aid, CPR, crisis intervention, and ongoing training.
