$1,000 Sign on Bonus

Schedule: Tuesday‑Saturday, 1st shift, 8‑hour shifts.

What you will be doing

Develop and implement individual treatment plans utilizing IM+CANS to maximize clients’ independent community living skills.

Complete documentation of mental health services in accordance with state and agency standards.

Collaborate with a team of behavioral health service providers to ensure a client‑centered approach.

Work may include remote components, subject to performance and operational needs.

Essential Job Responsibilities

Work scheduled hours and be flexible to meet client and program needs.

Provide independent program coverage when scheduled.

Ensure compliance with all agency, state, and federal regulations.

Facilitate communications and coordination of services with AID staff and community professionals.

Maintain professionalism and good boundaries with clients, coworkers, and external agencies.

Meet minimum service hour productivity standards monthly.

Complete all required case management documentation (IM+CANS, consents, paperwork) timely.

Sign all MRO documentation within 48 hours using Cx360.

Meet with assigned clients regularly, submitting daily activity logs.

Develop, review, and revise IM+CANS, correcting within the allotted timeframe.

Assist clients with activities of daily living, including apartment maintenance and training.

Guide MHP and RSA staff on implementation of IM+CANS goals/objectives.

Obtain input from clients, families, guardians, and staff to improve services.

Monitor and report on client safety.

Acquire and maintain required trainings and certifications.

Assist clients with bill payment, budgeting, and banking.

Oversee/ manage client funds when necessary.

Obtain and maintain client benefits (Social Security, Medicaid, etc.).

Maintain accurate client records per agency procedures.

Complete authorizations and reauthorizations promptly.

Update Cx360 with corrections annually.

Provide services aligned with client rights and the Mental Health Recovery Model.

Attend monthly clinical supervisions and team meetings.

Recognize emergencies and take appropriate action, completing necessary paperwork.

Promote relationships between clients’ families and additional supports.

Provide transportation to clients when necessary.

Coordinate community services to meet client needs.

Participate in planning/implementation of client activities.

Perform all other related duties as assigned.

Promote client wellness through coaching, counseling, and informational services.

Communicate with service partners to facilitate client wellness objectives.

Utilize educational materials for wellness coaching.

Promote awareness and respect for client diversity.

What will you bring to the table?
Education: Bachelor’s degree in Human Services or related field required for MHP; Master’s degree required for QMHP.

Experience: Experience in social services required; crisis management experience preferred; bilingual in Spanish preferred.

Physical: Ability to navigate stairs and provide services in clients’ homes; no lifting restrictions.

Equipment: Computer with Microsoft Windows applications, copy machine, telephone with voicemail, basic household appliances.

Additional requirements: Use of personal automobile, valid driver’s license, liability insurance as per AID personnel policy; must acquire and maintain certifications in first aid, CPR, non‑violent crisis intervention, CEUs and IM+CANS. Evening hours may be required. Must maintain vehicle and certifications.
