We are looking for a Case Manager Mental Health Professional in Elgin, IL who exemplifies that mission and wants to work for an organization that makes a difference.
The Case Manager MHP/QMHP is responsible for developing and implementing individual treatment plans utilizing the IM+CANS to maximize the client's independent community living skills.
The Case Manager MHP/QMHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable.
The Case Manager is expected to work with a team of behavioral health service providers within the agency as well as outside agencies, if applicable, to ensure a positive client‑centered approach.
This job may have remote components; remote work arrangements are discretionary and subject to performance and operational needs.
Essential Job Responsibilities
Work scheduled hours and be flexible to meet client and program needs, as assigned by Program Manager or Director. Provide independent program coverage when scheduled.
Ensure compliance with all agency, state, and federal regulations while completing assignments and updating policies.
Facilitate communications and coordination of services with other AID staff and community professionals via phone and email.
Maintain professionalism and good boundaries when working with clients, coworkers, and outside agencies.
Meet minimum service hour and productivity standards (MRO) monthly.
Complete all required case management documentation (IM+CANS, consents, residential/agency paperwork) on a timely basis.
Meet with assigned clients regularly based on program and client needs; submit daily activity logs.
Assist clients with activities of daily living, apartment maintenance; provide training consistent with treatment goals.
Obtain input from clients, families, guardians, and staff to improve residential services.
Help clients with paying bills, budgeting, banking; oversee or manage client funds when necessary.
Obtain and maintain client benefits (Social Security, Medicaid, Link Card, etc.).
Assure client records are properly maintained per agency procedures.
Provide services aligned with the Mental Health Recovery Model and client rights.
Recognize emergency situations and take appropriate action; contact Manager/Director and complete necessary paperwork.
Promote relationships between clients’ families and additional supports.
Provide transportation to clients in personal or agency vehicles when necessary.
Coordinate community services to meet client needs (financial, legal, advocacy, medical, etc.).
Plan and implement client activities.
Promote client wellness through coaching, counseling, consultation, motivational enhancement, and referral services.
Communicate with service delivery partners (physicians, nurses, counselors, pharmacists, employers, community supports) to facilitate client wellness objectives.
Utilize a variety of resources and educational materials for wellness coaching (e.g., Health Matters Curriculum, smoking cessation materials, weight management information).

What will you bring to the table?
Bachelor’s degree in human services or a related field (required) – MHP rate.
Master’s degree in human services or related field (required for QMHP role) – QMHP rate.
Experience working in social services (preferred).
A valid Illinois driver’s license and automobile insurance.

Additional Requirements
Use of personal automobile, valid driver’s license, and minimum liability insurance as defined by AID personnel policy.
Drive self and clients in agency or personal vehicle.
Acquire and maintain certifications in First Aid, CPR, non‑violent crisis intervention training, CEUs, and other relevant trainings.
