Case Manager, Mental Health Professional, Crisis Care Counselor, Mobile Join the Association for Individual Development (AID), a non‑profit dedicated to empowering individuals facing physical, developmental, intellectual, or mental health challenges, trauma survivors, and people at risk. We are seeking a dedicated Case Manager, Mental Health Professional, Crisis Care Counselor, Mobile who shares this mission and wants to make a meaningful impact in the community.
Schedule Monday, Tuesday, Saturday & every other Friday mornings 6 am‑6 pm. Flexible scheduling is required to meet client and program needs, including evenings, overnight, weekends, and rotating holidays.
Responsibilities Provide crisis intervention, de‑escalation, emotional support, and suicide risk assessment to callers via AID’s 24/7 crisis line.
Manage and assess Mobile Crisis referrals from 988, local police, and other community services.
Deliver in‑person Mobile Crisis Response throughout Kendall and Southern Kane County.
Conduct in‑person assessments to evaluate suicide risk, care needs, and access to support; develop safety plans with clients and collaterals.
Collaborate with internal and external service providers to ensure comprehensive client care and community stabilization.
Maintain accurate documentation for all phone and community interventions.
Attend scheduled work hours and remain flexible to respond to calls from shift start to end; provide line coverage when not on mobile response.
Consult with families, case managers, and other providers to offer effective individual crisis counseling and intervention assessments.
Meet monthly productivity standards and complete clinical assessments per funder and administrative requirements.
Provide telephone counseling using active listening and motivational interviewing techniques.
Complete required billing, MCRT daily logs, dispatch logs, 988 follow‑ups, and iCarol contact logs within specified deadlines.
Participate in monthly clinical supervision and additional training as assigned.
Develop and implement crisis safety plans and resource follow‑ups in accordance with Rule 132 and accrediting bodies.
Offer case management follow‑up to support linkage to services.
Maintain records in compliance with Rule 132 and conduct regular quality assurance checks.
Communicate effectively with agency staff, providers, families, and others to meet client needs.
Navigate Cx360 and crisis line documentation systems accurately.
Recognize emergency situations and take appropriate action, completing incident reports and case notes.
Participate in training and development opportunities; promote mental health awareness internally and externally.
Adhere to all Program 590 guidelines under the 590 Mobile Crisis Response Grant and IDPH Department of Mental Health.

Education Bachelor’s degree in human services or a related field (required).
Knowledge of DSM‑V (or current edition) and Rule 132.
Bilingual in Spanish preferred.
Experience At least one year of experience in behavioral health (preferred).
Physical Requirements Ability to communicate in person, on the phone, and in group settings.
Frequent standing, walking, stair navigation.
Capacity to provide services in clients’ homes and the community.
Driving and overnight work required.
Equipment Standard office equipment, including computer, phone, copier, and filing cabinets.
Worksite may involve off‑site community settings.
Additional Requirements Flexibility to work evenings, overnight, weekends, and rotating holidays.
Ability to work rotating on‑call shifts and cover non‑scheduled shifts as needed.
Reliable transportation with valid driver’s license and required automobile liability insurance.
Capability to drive both self and clients in agency or personal vehicles.
Demonstrated cultural competence and community awareness when working with diverse populations.
May be required to work at multiple locations.
Complete required training within 30 days of hire, including First Aid, CPR, Non‑Violent Crisis Intervention, CEU credits, and other relevant sessions assigned by the manager.
