Case Manager Mental Health Professional BH
Join Association for Individual Development (AID), a nonprofit dedicated to empowering individuals with physical, developmental, intellectual, and mental health challenges. We are looking for a Case Manager – Mental Health Professional (MHP) located in West Chicago, DuPage County.

$1,000 sign‑on bonus after 30 days

What you will be doing

Developing and implementing individual treatment plans using IM+CANS to maximize clients’ independent community living skills.

Completing documentation of mental health services in accordance with state and agency standards.

Working with a team of behavioral‑health service providers to ensure a positive, client‑centered approach.

Promoting client wellness through coaching, counseling, consultation, motivational enhancement, and information and referral services as needed.

Essential Job Responsibilities

Work scheduled hours with flexibility to meet client and program needs.

Provide independent program coverage when scheduled.

Ensure compliance with all agency, state, and federal regulations and follow updated policies and procedures.

Coordinate services with other AID staff and community professionals via phone and email.

Maintain professionalism and good boundaries with clients, coworkers, and external agencies.

Meet minimum service hour standards (MRO) monthly and complete all required case‑management documentation on time.

Assist clients with activities of daily living and provide training consistent with treatment goals.

Promote community/family involvement and client rights, and support crisis management as required.

Maintain accurate client records, benefits, and authorizations in Cx360.

Attend monthly clinical supervision, team meetings, and agency committees.

Provide transportation to clients when necessary and comply with all policy procedures for client outreach.

Perform all other related duties as assigned by AID policies and philosophies.

Education
Bachelor’s degree in Human Services or related field required.

Experience
Experience working in social services required.

Basic household appliances

Additional Requirements

Use of personal automobile and valid driver’s license with minimum liability insurance as defined by AID’s Personnel Policy

Drive self and clients in agency or personal vehicle as needed

Maintain certifications in First Aid, CPR, Non‑violent Crisis Intervention, and related CEUs

Evening hours may be required

Must maintain IM+CANS certification
