Overview The Association for Individual Development (AID) is a non-profit organization serving individuals with developmental, intellectual, physical and/or mental health challenges, those who have suffered a trauma, and those at risk. Our mission is to empower people with physical, developmental, intellectual, and mental health challenges to enjoy lives of dignity and purpose. We are looking for a Case Manager – Mental Health Professional (MHP) in McHenry County who exemplifies that mission.
What will you be doing? The CM MHP is responsible for developing and implementing individual programs to maximize clients’ independent community living skills. The CM MHP must complete daily documentation of mental health services provided in accordance with state and agency standards as applicable. The CM MHP is expected to work with a team of behavioral health service providers within the agency to ensure a positive client-centered approach.
Essential Job Responsibilities Work scheduled hours and be flexible to meet client and program needs, as assigned by Program Manager or Director. Includes cross training and coverage in other programs.
Responsible to provide independent program coverage when scheduled.
Assure compliance with all agency, state and federal regulations while providing services and completing assignments. Review and follow updated policies and procedures.
Facilitate communications and coordination of services with other AID staff and professionals in the community.
Maintain professionalism and good boundaries when working with clients, coworkers and outside agencies.
Meet or exceed minimum service hour provision productivity requirements as specified by program manager.
Complete all required case management documentation (IM+CANS, consents, program paperwork) on a timely basis.
Complete and sign all MRO Documentation within 48 hours using Cx360.
Meet with all assigned clients on a regular basis depending on program and client needs; turn in completed activity logs on a daily basis.
Complete assessment tools and necessary paperwork (IM+CANS, LOCUS, consents, client rights, and privacy practices) per program procedure.
Develop and provide direct implementation of IM+CANS for assigned caseload.
Assist clients with activities of daily living, including apartment maintenance, and provide client training consistent with the IM+CANS.
Assist in the training of new staff.
Monitor and report safety of clients’ living arrangement.
Acquire and maintain required certifications and trainings as well as other mandated trainings.
Oversight or management of client funds when necessary (dual signature accounts, AID rep. Payees).
Obtain and maintain client benefits (Social Security, Medicaid, Link Card, Ect).
Assure client records are properly maintained per agency procedures.
Update Cx360 with corrections whenever necessary, but at least annually to ensure accuracy of records.
Provide effective services for clients’ individual needs and in line with client rights and the Mental Health Recovery Model.
Attend monthly clinical supervisions per DHS requirement.
Attend monthly staff meetings and be a positive contributor.
Attend monthly Full Circle Community meetings and be a positive contributor.
Recognize emergency situations and take appropriate action. Contact Manager and Director per procedure. Complete necessary paperwork correctly (incident reports, petitions, case notes).
Be available for 24 hours per week for “walk-in” hours per the Full Circle Community contract.
Develop and implement activities and groups on a daily basis for Pearl residents.

Qualifications Education Bachelor’s degree in Human Services or related field required.
License/certifications Preferred certification in diverse and dual diagnosis.
Experience Experience working in social services preferred.

Additional Requirements The use of a personal automobile, a valid driver’s license, and the minimum amount of liability insurance as defined by AID’s Personnel Policy
Drive self and clients in agency or personal vehicle
Must acquire and maintain certifications in First Aid, CPR, Non-violent crisis intervention training, CEUs and other relevant trainings
Must be able to work Monday-Friday 8/9am-5/6pm and must be able to work one late day
Must be able to drive a passenger vehicle
Must maintain IM+CANS certification
Will be required to work in multiple AID and residential locations
