Position: Case Manager
Reports To: Clinical Supervisor
Overview CAMBA is a community-based organization offering more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. Magnolia House serves 200 single women in Brooklyn who are primarily diagnosed with mental illness, some with chemical addictions. The role focuses on supporting clients to achieve housing stability and independent living.
What the Case Manager Does Conduct initial intake, psychosocial evaluation, and assessment of clients’ needs; perform periodic re-assessments of independent living plans including short-term and long-term goals
Create and maintain client electronic and paper files
Assist clients in attaining goals by identifying barriers, connecting to community resources, and referring to appropriate services (on-site medical, psychiatric services, etc.)
Advocate for clients to gain housing stability
Recommend and implement strategies to engage clients in the process toward permanent housing
Monitor progress toward goals through regular face-to-face contacts/sessions
Document and update all client encounters via progress notes in DHS CARES Database and printout notes for client files
Follow up with clients and referral organizations regarding progress
May act as client liaison/advocate with outside organizations on education, healthcare, housing, legal issues, etc.
May schedule and escort clients to appointments with referral organizations
May assist clients in completing applications for benefits and entitlements
Minimum Education/Experience Bachelor’s degree and/or equivalent experience
Other Requirements May be required to become First Aid/CPR certified
May be required to become certified in overdose prevention
Compensation $42,000-$46,000 annually
Note: This salary range reflects the base range for the position. Final offer is based on factors including experience, education, budget, program size, internal equity, and other relevant considerations.
Status and Benefits Status: Full-time (35 hours per week)
