Hourly Rate: $27.47 – $32.96

Overview
We are seeking an experienced Case Manager for our new men’s shelter, Liberty Plaza. The Case Manager is part of an interdisciplinary team dedicated to stabilizing shelter clients by improving their independent living skills and assisting clients in progressing toward housing goals. The role involves assessing client needs, linking clients to services, planning paths to stabilization and re‑housing, monitoring progress, and documenting all client interactions.
Essential Functions
Assess clients’ treatment needs and develop, monitor, and evaluate treatment plans.
Admit new clients by reviewing records and applications and conducting orientations.
Conduct intake interviews to determine mental and medical needs.
Establish treatment programs by setting schedules, coordinating services, and arranging resources (including transportation and escort).
Monitor cases by verifying attendance, evaluating treatments, advocating for services and entitlements, intervening in crises, and providing personal support.
Communicate clients’ progress through weekly interdisciplinary meetings and evaluations; disseminate results and obstacles to the therapeutic team and family; identify treatment influences.
Improve treatment results by studying, evaluating, redesigning processes, implementing changes, and rewriting policies and procedures.
Required Qualifications
High school diploma, HSE, or GED (required).
Minimum of 2 years of social services experience (required).
Possession of a valid Food Handler’s License, or the ability to obtain one within six months of hire (required).
Experience working with the homeless, mentally ill, substance abuse, and/or low‑income populations (required).
Strong relationship‑building skills (required).
Strong computer skills, including Word and Excel (required).
Ability to work evenings and/or weekends on a rotating schedule (required).
Preferred experience working in a homeless shelter (preferred).
Knowledge of mental health, substance use, or homeless populations (preferred).
Bilingual (English/Spanish) preferred.
Work Environment
This job operates in a shelter environment and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.
Physical Demands
The employee is regularly required to stand, walk, sit, stoop, climb stairs, and kneel. Occasionally the employee will have to lift and/or move up to 10 pounds.
