Field: This role is primarily field-based, involving travel to client sites or designated locations, with occasional office attendance for meetings or training. This ensures flexibility and direct engagement with clients.
The Case Manager- LCSW/LSW/LMSW/LMHC/LMFT (also called the LTSS Service Coordinator - Clinician ) is responsible for managing members' cases under the IN PathWays for Aging program, including assisting with assessments, coordinating care, and ensuring member needs are met within the scope of licensure.
How You Will Make An Impact
Assist RN in identifying high-risk members.
Obtain clinical data as directed.
Identify members who may benefit from alternative care or waiver programs.
Provide collected information to the RN for verification and care plan development.
Coordinate care for members with chronic conditions, in collaboration with the healthcare team.
Apply clinical guidelines to decision-making, without requiring nursing judgment.
Minimum Requirements
LSW, LCSW, LMSW, or other licensed in accordance with state law, with at least 2 years of experience working with individuals with chronic illnesses or disabilities.
Current, unrestricted license in applicable state(s).
Preferred Skills and Experience
Experience working with older adults in care management or related roles.
Experience managing community or facility-based care management caseloads.
