The Case Manager/Intake Coordinator is responsible for processing applications received for the Residential Treatment Program, providing initial patient orientation, maintaining accurate patient files, and ensuring proper upkeep of patient information. The position serves as a case manager for individuals with mental health and substance use disorders needing access to medical, behavioral, social, and other services tailored to their needs. This role is specific to the Kanvwotiyi Residential Program.

Essential Duties and Responsibilities

Perform case management assessment, person‑centered planning, referral, linkage, and ongoing monitoring.

Process applications for the Residential Treatment Program, coordinate interviews, schedule intake appointments, and orient patients to the program.

Maintain and discharge patient files in line with CIHA policies and procedures.

Document all treatment and clinically significant events in the Electronic Health Record following CIHA standards.

Gather comprehensive information (medical, psychosocial, behavioral, financial, cultural, etc.) and incorporate it into the EHR.

Engage patients in person‑centered planning to acquire desired outcomes and symptom reduction.

Link clients to needed services and support systems.

Provide timely status updates to the treatment team and utilize crisis intervention protocols as needed.

Collaborate with medical staff and other agencies to facilitate patient care, discharge, and follow‑up.

Participate in clinical staffing, treatment plan discussions, and program improvement meetings.

Offer written, telephonic, and face‑to‑face consultation to individuals and organizations affecting the patient (e.g., schools, courts, probation, physicians, hospitals).

Conduct community and home visits when required.

Represent CIHA at team and emergency meetings.

Support Tribal Option Care Managers and adapt to additional duties as assigned.

Education, Experience, and Qualifications

Bachelor’s degree in a human services field or equivalent experience.

Experience working with substance use disorder and/or mental health populations.

Valid driver’s license.

Preferred: experience serving Native American populations and familiarity with Cherokee culture.

Two years of experience with the target population is favored.

Demonstrated recovery‑oriented philosophy.

Job Knowledge and Skills

Knowledge of crisis intervention techniques and ability to assess potential lethality.

Proficiency with electronic health record systems and record‑keeping.

Strong problem‑solving, communication, and teamwork abilities.

Understanding of behavioral health principles, community resources, and treatment modalities.

Capacity to establish rapport and engage patients.

Willingness to learn about Cherokee culture and its relevance to care.

Other Relevant Information
Position responsibilities include ongoing continuing education through in‑service training and approved workshops. The role requires maintaining professional boundaries, supporting team collaboration, and ensuring data accuracy in accordance with CIHA policy. Primary work setting is a hospital or outpatient clinic with occasional home visits; some travel and early‑morning/late‑evening hours may be required.
