Overview
The incumbent performs nursing procedures to patients of all ages independently within the framework of established policy. The Registered Nurse is a licensed professional nurse responsible for assessment of patients, development, implementation and evaluation of a patient plan of care; administration of therapeutic measures; teaching and counseling of patients and families; and participating in the development of nursing policies and procedures and quality assurance standards. The Registered Nurse is responsible for the delivery of comprehensive health care to patients of all ages, utilizing standards of care and practice, policies, protocols, and procedures. The purpose of the work is to perform the full range of the nursing process from assessment to evaluation. The work affects the physical and psychosocial well being of the patients and their families.

Responsibilities

Utilizes best practice models to identify, incorporate or develop best practices for panel management. Collaborates with other teams to share and establish best practice for health promotion and disease prevention strategies.

Manages panel by addressing and resolving acute care needs and chronic care needs through a team-based approach.

Leads their pod and assists the Primary Care Provider in panel management to meet facility, national, and regulatory quality indicators.

Utilizes iCare to track, monitor, and assure the appropriate follow-up of clients targeting specific indicators.

Uses the care management platform for documentation of care management functions (care needs screening, comprehensive assessment, and care planning) and leverages dashboards for population health interventions.

Utilizes NC Health Connects for information gathering and data collection for management of care needs or gaps in care.

Coordinates and follows up on referrals to outside specialty providers, recent ED visits, and ICC visits; ensures timely availability of treatment notes to the PCP.

Participates in continued development of the Case Management role in the Patient Centered Medical Home (PCMH) and Advanced Medical Home (AMH/AMH+).

Provides standard nursing care for patients throughout the age continuum.

Develops, implements and evaluates care plans; promotes health care outcomes with current clinical practice guidelines.

Provides patient education, advice and information on health assessment, disease processes, medications, treatment plans and available community resources.

Assesses patient needs using established guidelines, protocols, and pathways; provides appropriate follow up as directed or per guidelines.

Performs nursing care in routine and specialty clinics, curative, rehabilitative, and preventive care; may participate in emergency care as needed.

May function as charge nurse when assigned; assigns staff according to competency and patient complexity, providing supervision and problem resolution.

Develops individualized plan of care with input from the patient, family, and pod members for high-risk patients.

Initiates care plans based on assessment of illnesses, injuries, diseases, social determinants of health (SDoH), and human behavior; adheres to standards of care.

Implements nursing care according to the established health care plan; may adjust work schedule for late clinics or on-call requirements; may rotate between departments including inpatient, satellite clinics and emergency department.

Participates in cross-training of services within the nursing department.

Performs nursing interventions including detecting changes in condition, initiating appropriate actions and resuscitative measures when needed; documents observations and communicates with physicians and staff.

Administers and documents medications per guidelines; records known drug allergies and ensures timely execution of treatments and tests.

Provides counseling, teaching, and anticipatory guidance to patients and families; documents education activities.

Coordinates health care needs through referrals to support services, community, and tribal resources.

Maintains a safe environment, follows Universal Precautions, medical waste management and OSHA regulations.

Participates in performance improvement activities and risk management reporting.

Ensures supplies are adequate and equipment is functional; maintains clean and orderly work areas.

Promotes patient advocacy and confidentiality; serves on committees as appointed.

Maintains positive communications within Nursing and other departments and with tribal/community agencies to coordinate goals and objectives for the served population.

Functions appropriately during disasters and maintains currency/competency in nursing practices through ongoing training.

Evaluates patient care and revises care plans as patient needs change; documents nursing care and progress per hospital policy.

Participates in ongoing nursing quality assurance program.

Education, Licensure, Certification, and Experience

Bachelor or Associate Degree in nursing from an accredited program required. Minimum of two years of nursing experience preferred.

Unrestricted valid Registered Nurse license within North Carolina or a reciprocal state.

Current Basic Life Support (BLS) required; can be acquired within 6 months of appointment.

Valid North Carolina driver’s license.

Knowledge of community resources and health care programs; ability to establish, monitor, and evaluate standards for nursing practice.

Knowledge and ability to operate specialized medical equipment and interpret monitoring data.

Knowledge of legal and ethical aspects of nursing.

Complexity of Duties
The incumbent leads a pod, exercises leadership, and maintains integrity, poise, and control. Provides comprehensive nursing care, assesses needs, develops plans, administers treatments, operates equipment, evaluates care, and educates patients and families. Uses professional standards and hospital policies to guide decisions and applies independent judgment in applying guidelines to individual situations.

Supervision Received
Plans, schedules, and provides nursing care under the Outpatient Administrator; evaluates work for technical soundness and adherence to professional standards.

Responsibility for Accuracy
The incumbent’s performance affects patient health and recovery. Follows policies and procedures; errors can impact patient outcomes. Clinical actions are verified by the supervisor or systems checks.

Interactions with patients, families, hospital personnel, and community agencies to exchange information and provide education. Maintains confidentiality and professional conduct in all communications.
