Overview

The Case Manager II (CMII) coordinates and facilitates patient care across a multidisciplinary care continuum within a clinical service unit structure. The CMII supports, while providing psychosocial assessment and supportive counseling to patients and families to facilitate clinical improvement and discharge. Services provided include: psychosocial assessment; advance directive counseling; chemical dependency screening and referral; processing of guardianship issues; identification and crisis intervention for victims of abuse/neglect; provides knowledge and referrals to available community resources. All activities are focused on assisting the patient, family, and team to address psychosocial issues to ensure referral and mobilization of available community resources.

Education

Qualifications

Bachelor’s degree in the field of human services, psychology, and sociology, or social work or related healthcare field required

Licensure/Certification/Registry

Illinois Licensed Social Worker required obtained within two years of hire; Certification in Accredited Case Manager (ACM) and/or Chronic Care Management (CCM) preferred

Experience

Five to seven years of inpatient case management experience

Other Knowledge/Skills/Abilities

Good communication and interpersonal relationship skills
Able to organize work with minimal supervision
Able to focus attention to minute details
Access, Word and Excel computer skills required

Maintains appropriate documentation
Assesses and evaluates patients’ ability to provide self-care; develops discharge plan and goals of care with patient and involved caregivers.
Assists with making referrals to related agencies taking into consideration freedom of choice.
Provides information to social service/discharge planners to facilitate discharge planning process.
Participates in multidisciplinary rounding and care conferences with other departments.
Serves as a resource person to physicians and hospital staff in the appropriate use of health care resources.
Serves as a liaison with nursing and medical staff and community agencies to facilitate problem solving and to coordinate services.
Assesses and develops plan early upon admission and revises as needed depending on needs of the patient.
Precepts new colleagues to case management team. Delivering structured instruction on case management practices, mentoring through real time support and feedback, monitoring performance, and ensuring adherence to MH protocols.
Assists in the development, review, revision and audits of case management policies and procedures to ensure compliance with best practices and regulatory compliance.
Supports leadership in education, changes and advancements in the field of case management.
Intervenes on a regular basis to manage family expectations and facilitates their decision-making process.
Participates in unit-based councils to further the case management practice. To ensure collaboration across multidisciplinary teams.
Accepts on-call as scheduled and responds as needed.
Takes responsibility for upgrading professional skills and knowledge.
Serves as a super user for EHR and other case management tools.
Demonstrates clinical competency by completing BLS CPR and other training modules for colleagues assigned by leadership.
Performs other duties as assigned.
