For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Program provides free English classes for individuals interested in learning the language. Students receive social services and legal referral information about U.S. citizenship and other immigration services. Trainings such as OSHA and First Aid help individuals in creating resumes and cover letters. At times, Mock Job Interviews are provided to individuals allowing them to develop interview strategies as well as to enhance communication skills.
STATEMENT OF THE JOB Under the direction of the Project Director, the Case Manager I renders assistance to ESOL students as an FDC Coach through comprehensive case management services and education/career counseling. The Case Manager I works in collaboration with the Project Director and internal agency service providers.
Target, engage, and conduct outreach to students.
Provide case management services, assessment and intake of students to determine eligibility for services, including responding to telephone and walk-in inquiries, conduct pre-screenings and provide brief and full services.
Carry a caseload required to meet funding source targets and keep client data base up to date.
Maintain case records and program documentation in accordance with agency and program guidelines.
Work with the student to assess strengths, needs and resources; develop an individualized service plan (ISP) to meet short- and long-term goals; and follow up to determine whether goals have been met and/or needs have changed.
Advocacy, linkage, and escort services for students benefit.
Conduct periodic assessments of enrolled students to determine progress in literacy, employment and education and ISP goals.
Develop and manage an Individualized Family Service Plan coordinating with students, and internal agency and community resources.
QUALIFICATIONS Bachelor’s degree in social services or a related field or an associate’s degree in social services or a related field and Family Development Training credentials
One year of experience working with the City System such as Public Assistance, child welfare, education, and housing and FDC Certification
Knowledgeable of services for children and families
Bilingual (English/Spanish) Required.
Ability to work flexible hours (evenings) as assigned.
Able to conduct home/field visits and travel throughout the five boroughs as needed.
Ability to utilize public transportation, climb stairs, walk.
