CAMBA GARDENS Phase II is a 293 unit supportive/affordable housing development in East Flatbush, Brooklyn. 182 units are set aside for formerly homeless individuals and families; 108 for Population A individuals; 50 for Population H individuals; and 24 for Population II families.

What The Case Manager I Does Maintain professional relationships with clients and client confidentiality
Practice Universal Precautions/Standard Protocol & Procedures
Comply with Federal, State, City and CAMBA security and privacy policies for health information
Review documentation establishing tenants' eligibility and maintain files
Create and maintain tenant files
Conduct initial intake and assessment of tenants and their families, with periodic re-assessments
Collaborate with tenants to prepare independent living plans with short-term and long-term goals
Identify and locate community resources and make referrals to services both within and outside CAMBA
Assist tenants in attaining goals and advocating for themselves toward self-sufficiency
Recommend strategies to encourage participation in the process
Monitor progress via regular contact and visits; document via progress notes
Participate in case conferences
Follow up with tenants and referral organizations on contact and progress
Provide required information for weekly/monthly/quarterly/annual reports
May recommend closing of cases under specified conditions
Act as tenant liaison/advocate with outside organizations on education, healthcare, housing, legal issues
Schedule appointments with referral organizations
Escort tenants to appointments as needed
Assist tenants in completing applications for benefits and entitlements; may process applications on tenants’ behalf
Follow-up after primary goals are achieved to ensure stability
Evaluate living conditions through home visits
May prepare marketing materials for the program
May prescreen tenants by telephone for eligibility and schedule intake appointments
Input tenant data and progress information into automated systems
Minimum Education/Experience Required Experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience
Other Requirements Must be fingerprinted and obtain Criminal History Check (CHC) with the Justice Center for the Protection of Vulnerable People with Special Needs; maintain fingerprint clearance during employment
As a Mandated Reporter, immediately report suspected abuse/neglect to the Vulnerable Persons Central Register (VPCR) Hotline and complete a CAMBA Incident Report
Flexible hours may be needed to provide shift coverage until 8 pm
Compensation : $51,500 annually
Status: Full-time (35 hours per week)
