Position: Case Manager Location: 880 Willoughby Street, Brooklyn, NY 11221
Program: CAMBA’s Housing Ventures (CHV) – Anna Gonzalez Apartments
Reports To: Program Manager
Overview: CAMBA’s Housing Ventures is an initiative to develop affordable housing and provide comprehensive housing and supportive services. This role supports a 100-unit supportive housing building in Bushwick, Brooklyn, serving a mixed tenant population including low-income individuals and families, people living with mental illness, formerly chronically homeless, and individuals living with HIV/AIDS.
What The Case Manager Does:
Recommend and implement strategies to engage clients in the case management process.
Monitor clients’ progress toward goals via regular contact and visits; document progress notes.
Participate in case conferences and coordinate with referral organizations.
Prepare and submit required information for weekly, monthly, quarterly, and annual reports.
Evaluate cases for closure based on goal attainment, participation, or eligibility changes.
Act as client liaison/advocate with outside organizations on education, healthcare, housing, legal issues, etc.
Schedule appointments and escort clients to required appointments as needed.
Assist clients with applications for benefits and entitlements, including processing where appropriate.
Follow up after primary goals are achieved to ensure client stability.
Conduct home visits to evaluate living conditions when appropriate.
Support program marketing efforts as needed and prescreen clients for eligibility.
Enter and maintain client data and progress information in the automated database.
Perform other duties as assigned, which may evolve over time.
Minimum Education/Experience Required:
Bachelor’s degree (e.g., B.A., B.S.W.)
Other Requirements:
Flexible hours may be needed occasionally to provide shift coverage until 8 pm
Compensation: $40,000–$45,350 annually
Note: Salary ranges represent the low and high ends for the applicable position and program. Salary offered is based on factors such as experience, education, budget, program size, internal equity, and other relevant factors.
Status: Full-time (35 hours per week)
