Overview Case Manager - Full-Time/Part-Time All Shifts at North Shore Health. Mileage reimbursement provided.
RN Case Manager - Hospice. Full-time hours; locations in Milwaukee areas, Sheboygan areas, and Central Wisconsin. Training available; prior hospice experience preferred but not required for RN—training provided.
Responsibilities Assist the admission nurse in coordinating timely admissions and provide hands-on clinical nursing in collaboration with patients, families, physicians, the Interdisciplinary Team, and other health care professionals involved in the patient’s care.
Assess and coordinate a pain/symptom management program with input from the patient, family, attending physician, and Interdisciplinary Team, using pharmacological and non-pharmacological methods to enhance quality of life.
Facilitate freedom of choice, relief of suffering, and dignity in end-of-life care; educate patients and families on disease processes and progression.
Provide support and education for families per the hospice philosophy of care, addressing physical, psychological, spiritual, and financial comfort.
Participate in Interdisciplinary Team meetings and update the Plan of Care (POC) based on identified problems and needs; ensure care aligns with the POC and appropriate level of care.
Coordinate medication management with PCP and medical director to prevent polypharmacy and optimize symptom management.
Coordinate services with visiting nurses, home health aides/homemakers, chaplains, social workers, volunteers, and other staff; order medications, Durable Medical Equipment, and supplies as needed.
Provide consultation and supervision to CNAs and LPNs; participate in performance evaluations and orientation for clinical staff.
Support cost-effective practices, participate in QAPI activities, and uphold the Hospice Code of Ethics.
Triagescheduling and symptom management calls from IDT members; inform Clinical Director of changes in patient condition that require a level-of-care adjustment.
Qualifications Required Education And Experience
Registered Nurse graduated from an accredited program; currently licensed and in good standing with the Wisconsin State Board of Nursing.
Preferred Education And Experience
One to two years of nursing experience in an acute care setting and 1 year of experience in home health, hospice, or comparable settings is preferred.
Demonstrated aptitude for hospice philosophy and comfort care; ability to work cooperatively with social workers, chaplains, CNAs, LPNs, and volunteers.
Work Environment The responsibilities involve significant physical activity (standing, lifting, bending, stooping, pushing, pulling, twisting). Employee must be a licensed driver with a reliable vehicle and maintain appropriate insurance.
