Salary: $23.00/hr – $30.00/hr; Signing bonus: $1,500.

Role Overview
The Case Manager delivers case management services to assigned clients, collaborating with the care team to provide ongoing care coordination and treatment planning. Responsibilities include developing medically necessary treatment plans, guiding treatment planning efforts using the Golden Thread, DLA‑20 assessment, and Motivational Interviewing.

Key Responsibilities

Meet with new clients and families to explain the model of care and introduce available services.

Conduct the DLA‑20 Assessment to determine case‑management goals and collaborate with clients and families on a medically necessary treatment plan.

Provide case management and care coordination services according to the treatment plan and support level of care needs.

Support clients in developing skills to improve functioning in activities of daily living (personal hygiene, grooming, nutrition, grocery shopping, budgeting, housing).

Achieve 60% productivity (24 hours) billable services per full FTE.

Identify and link clients to community services (primary care, mental health/substance abuse treatment, housing, financial benefits).

Facilitate educational and skill‑building group treatment services to support clients in managing chronic health conditions.

Provide crisis intervention as needed.

Qualifications
Education: BA Degree in Behavioral Sciences and at least one year of full‑time equivalent clinical experience.

Experience:

Crisis response and verbal de‑escalation.

Treatment planning.

Ability to work effectively with diverse clients using a strengths‑based approach.

Knowledge, Skills & Abilities:

Understanding of Medicaid and Medicare benefit packages.

Proficiency in utilizing multiple platforms.

Additional Qualifications:

Ability to obtain Washington State Agency Affiliated Counselor Registration upon hire.

Independent worker.

Current driver’s license and automotive insurance (transportation of clients).

Pass required background checks.
