Orange County Family Wraparound – NAFI New York Middletown, Orange County, NY is a short-term comprehensive General Preventive Services program focusing on community supports; early interventions; home-based responses; and post-placement re-integration for children. The program includes a strong clinical component for family and individual treatment. All Wraparound program services take place within the community and include case management, in-home therapy, and respite services.
Family Wraparound Services is currently exploring candidates for a dynamic Case Manager!
Responsibilities Provide support and guidance to the program families and youth around service plan goals. Primarily responsible for providing linkages to community services to the family to ensure that they receive benefits, entitlements and needed services.
Provide case management counseling to youth in the program as it relates to service goals, through meeting with the family at minimum twice monthly in a community location. Assists with group activities, with childcare during support groups, and with respite, tutoring and mentoring when needed.
Provide transportation support for families to attend medical appointments and/or court, as needed. Timely completion of documentation in accordance with NAFI and OCFS standards and policies.

Qualifications Must hold a valid driver’s license, have own vehicle and willingness to travel throughout Orange County and to travel periodically to offices in Westchester County, and other locations as required to fulfill job responsibilities.

Bachelor’s degree with 1-3 years of experience and/or training; or equivalent combination of education and experience
This position directly reports to the Program Supervisor.
