Position Summary
Full‑time Case Manager – Starting at $44,676 – $47,650 contingent upon experience, education, etc. TASC Case Manager will provide comprehensive and specialized case management services for adults and youths with identified behavioral health needs referred to TASC from multiple systems. The role includes direct services and referrals based on identified needs of individuals enrolled in TASC services, statewide with location determined by the work site.

Essential Duties and Responsibilities

Assess, evaluate, facilitate, and monitor individuals’ progress in relation to established goals and objectives.

Complete intake packets for program enrollment and participation.

Provide referrals and crisis intervention for the individuals’ identified needs.

Interview individuals, review records, and confer with other professionals to review individual progress and determine suitability for program participation.

Have knowledge of DSM‑V and ASAM criteria.

Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and engage in integrated staffing with individuals receiving TASC services.

Identify and refer individuals with behavioral health needs.

Complete and submit internal and external program reports.

Maintain all required documentation, including service plans, reports, progress notes, discharge summaries, and other individual‑related activities.

Monitor all individuals’ progress towards service goals.

Monitor the TASC database system and contractual/external database systems to ensure individual enrollment.

Ideal Candidate

High school diploma or GED equivalent.

Valid driver's license.

CADC certification within two years of employment with TASC (may be required based on contractual obligations).

Proficiency in the Microsoft Office suite.

Experience in group facilitation.

Knowledge of ASAM placement criteria, DSM‑V relating to substance‑related disorders, and the Code of Ethics for substance abuse professionals, or the ability to acquire such knowledge.

Comfort working inside a correctional setting and with persons convicted of serious violent offenses.

Flexibility in thinking and approaches to leading group discussions.

Excellent verbal, written, and interpersonal communication skills.

Strong time‑management skills with a proven ability to meet deadlines.

The agency offers a hybrid work schedule that combines in‑office and remote work. Employees are required to report to the work site three days per week, with the option to work from home for up to two days remotely after 60 days of employment, contingent upon job duties.
