Position Summary The Case Manager-DSS Family Centered Program will be responsible for providing case management support, guidance and referrals to assist Ulster County Department of Social Services (DSS) clients residing in area hotels and motels with a goal toward housing stability.
The DSS Family-Centered Case Managers will provide a “warm hand off” to area housing providers and local landlords so that families are more likely to successfully transition from hotel/motels into housing and remain stably housed. This is a RUPCO position but will be seated at Ulster County DSS’s office located at 1061 Development Court, Kingston NY with drop in space at RUPCO’s office located at 289 Fair Street in Kingston.
Essential Responsibilities Provide case management support for DSS clients residing in area hotels/motels with a goal for housing stability
Collaborate with DSS staff on site to respond to client housing needs as they arise
Travel to area hotels and motels and conduct meetings with clients to assist with the completion of paperwork
Assist with the completion of housing applications and other related paperwork
Assist with resume development, and financial literacy including budgeting and planning
Make referrals and assist clients in securing mainstream benefits
Collaborate with other agencies that may offer identified needed services and supports
Track progress obtaining supports identified in goal planning
Responsible to ensure confidentiality of client files
Responsible for data entry of clients in all applicable databases
Prepare quarterly reports on numbers served, status and successful outcomes. Provide additional reports as required
Responsible for timely return of phone follow up with clients and providers
Some flexibility to work hours as business necessitates and to best serve the clients of the program

Health, Safety, & Compliance Maintain a safe work environment in a clean, organized, and safe manner. Conduct daily inspections of work areas to look for unsafe conditions
Actively participate in health and safety practices and accident prevention
Ensure all accidents are reported to supervisor and Human Resources, thoroughly investigated, and properly documented. Investigate near-miss incidents to determine root cause to promote accident prevention
Use all required safety devices and personal protective equipment
Agency Standards Demonstrate positive work attitudes and habits. Act as a role model for success for individuals
Maintain a clean and neat appearance consistent with agency standards
Attend all required meetings and trainings
Participate in training programs for professional growth as scheduled and approved
Responsible for understanding and adhering to RUPCO Inc.’s applicable program rules, regulations, and policies
Required Qualifications Education: Bachelor’s Degree in human services related field (social work, psychology, or mental health counseling) is highly recommended, High school diploma required
Experience: 3 years of experience working in the human services field and/or Case Management Experience is required, Experience with homeless, vulnerable populations preferred
Skills: Must have good communication and interpersonal skills and the ability to work with diverse groups, Computer literacy and data base experience in excel and access helpful, Must have current driver’s license and reliable vehicle for travel
Salary: $43,000 Annually
