Coordinate admissions, discharges, and transitions of care. Develop, monitor, and revise individualized care plans. Serve as liaison between patients, families, staff, and external healthcare providers. Facilitate care conferences and ensure compliance with regulatory standards. Monitor utilization of services and support cost-effective care delivery. Maintain accurate documentation in compliance with facility and state/federal guidelines.
Key Responsibilities:

Maintain accurate documentation in compliance with facility and state/federal guidelines
Qualifications:
RN or LPN license (preferred but may vary by facility)
Experience in case management or discharge planning, preferably in a SNF or long-term care setting
Strong communication, organization, and problem-solving skills
Knowledge of Medicare/Medicaid and insurance regulations
