Case Manager – Crisis Services – Goshen
Oaklawn Psychiatric Center invites applications for a Case Manager to provide crisis services in the Goshen area. This role is part of an organization that serves 27,000+ children, adolescents, and adults annually across northern Indiana, offering a continuum of mental health and addiction treatment.

What You’ll Do
Case Managers in Crisis Services deliver crisis interventions through walk‑in, hotline, and mobile response. The responsibilities include:

Providing immediate crisis support to adults in the community.

Coordinating care across service providers and community partners.

Developing individualized treatment plans and monitoring progress.

Assisting clients in accessing resources such as housing, food, medical care, transportation, and social services.

Documenting all services in the electronic health record.

Working closely with outpatient therapists, physicians, and other treatment providers.

Operating 12‑hour shifts, including weekends, as needed.

Primary Duties for Case Managers Include

Evaluating the need for services.

Developing and monitoring treatment plans that support client goals.

Linking clients to necessary services (e.g., food, housing, medical care, Social Security benefits, transportation, skills training).

Advocating on behalf of clients and coordinating with healthcare providers, employers, community agencies, and other Oaklawn services.

Documenting all services in the electronic record.

Providing client care based on the level of need.

Maintaining frequent communication with the client’s treatment team.

Minimum Hiring Requirements

BA/BS in social service or a related human services field required.

Two years of experience in a social service setting preferred.

Valid driver’s license and proof of insurance required.
