Position Overview
Northeast Family Services is a premier mental health provider with over 20 years of experience serving children, youth, and families. We are seeking a dedicated Case Manager to join our team, reporting to the Clinical Supervisor. The role involves serving as a liaison among the treatment team, clients, families, and collateral service providers, with positions available throughout New Hampshire.
Responsibilities
Maintain caseload and ensure proper implementation of in-home services per DCYF referrals.
Collaborate with DCYF on referrals, treatment plans, and documentation.
Develop treatment plans with measurable goals in partnership with families and DCYF.
Complete all required documentation, including assessments and reports.
Participate in court reporting and provide parent education, supervised visitation, and support services.
Coordinate with various agencies and assess client safety continuously.
Transport clients as needed, including installation of car seats.
Respond to on-call support needs and work flexible hours, mostly afternoons and early evenings.
Document clinical contacts via Electronic Health System.
Minimum Requirements
Bachelor’s Degree in social work, counseling, psychology, or related field.
At least two years of experience working with children, youth, and families preferred.
Valid driver’s license and access to a reliable vehicle.
