Overview Join to apply for the Case Manager role at Northeast Family Services .

Compensation: Salary up to $50k plus a $500 stay-on bonus after 6 months .
Responsibilities Maintains assigned caseload and ensures the proper implementation of in-home services in accordance with referrals made by DCYF.
Collaborates with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service-specific documentation.
Develops and collaborates on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.
Adheres to all documentation requirements of service delivery including assessment and report writing.
Participates in reporting progress to the court system through in-person appearances and written reporting as requested.
Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.
Offers parent coaching and support including effective parenting strategies and meeting healthcare and educational needs of children.
Enforces supervised visitation expectations as identified by DCYF to ensure child safety.
Interfaces with DCYF, NH court system, schools, behavioral health providers, and other treatment team members as necessary.
Assesses client risk on an ongoing basis and consults with clinical supervisor regarding safety concerns.
Transporting clients, including children and adults, requiring car seat and booster seat installation.
On-call rotation: respond to phone calls to provide family support as needed.
Evening availability is required; most visits are in the afternoon or early evenings. Schedule/hours are flexible to client needs.
Document all clinical contacts and documentation through the Electronic Health System.
Other duties as assigned.
Qualifications Bachelor’s Degree with a major in social work, counseling, psychology or related human services field.
Two years minimum experience working with children, youth and families in a home or community setting (preferred).
Valid driver’s license with reliable access to a vehicle.
